Chief complaint:  Patient was presented with altered sensorium since yesterday. Patient was apparently alright 3days back then he stopped using injection MIXTARD since 3days. Patient had history of irrelevant talk since yesterday but able to recognise people.  History of present illness:  Patient has low grade fever since yesterday ,intermittent in nature, relieved on medication.  Not associated with cold ,cough,vomiting, loose stools etc. No SOB, chest pain, palpitations,  edema or decreased urine output. This also reflects my patient centered online learning portfolio.  Case history :  A 50 years old male patient, resident of Mothkur,Telengana came to General medicine OPD on 2023.  CHIEF COMPLIANT: Patient complaince about No movement of both  left upper and lower limbs since 3 days  HISTORY OF PRESENT ILLNESS: Patient was apparantly asymptomatic 3 days back and then he noticed no movement in his left upper and lower limb and  associated with chest pain, shortness of breathing, difficulty in speaking. This also reflects my patient centered online learning portfolio.  CASE SHEET:  a  68 years old man with leg pain.  CHIEF COMPLAINTS: patient complaince of pain in his right knee and ankle since 2days and also associated with swelling  of foot since 1day HISTORY OF PRESENT ILLNESS: Patient was apparently asymptomatic 2days  back.Then he developed leg pain over right knee and ankle region due to hit by a rod.  Pain is continuos and throbbingg type of pain and pain is radiating towards ankle.  Pain is aggrevative during walking and no relieving factors. He also complains about over swelling at right ankel since1day. Swelling is soft and non tender and he is associated with small ulcer.
